The scope of home-based services is related to, but not limited to, those services funded by Oklahoma DHS-DDS (Department of Human Services, Developmental Disability Services). Whether you are looking for supported living, agency companion, and daily living support homes. With an additional array of options not mentioned or in development.
· In accordance with the needs of the member, Daily Living Supports and Agency Companion homes include hands-on assistance, supervision, or prompting so that the member performs the task, such as eating, bathing, dressing, toileting, transferring, personal hygiene, light housework, money management, community safety, recreation, social, health, or medication management.
· Daily Living Supports also include assistance with cognitive tasks or provision of services, per OAC 340:100-5-57, to prevent a member from harming self or others.
· Daily Living Supports also include: the provision of staff training per OAC 340:100-3-38, to meet the specific needs of the client.
Supporting families through short-term care, allowing consumers to develop and enhance their quality of life in and outside of the home. Encouraging personal responsibility, discovering personal interests, and developing friendships with others.

· Access to Community-based waiver services
· Provided licensed transportation
Serves the needs of individuals 18 years of age and older with intellectual disabilities who would otherwise require placement in an ICF/IID.
The In-Home Supports Waivers (IHSW) were created in 1999 in response to the continually increasing numbers of individuals with developmental disabilities remaining on the waiver waiting list. These capitated waivers allow individuals and families to select services necessary for each individual to remain in his or her own home or family home. Individuals on the IHSW are assigned to Developmental Disabilities Services (DDS) case managers to assist them in locating, securing, and coordinating needed services.
Services provided from 3 years of age to end of life for individuals for whom the IHSW is not sufficient to meet their needs. Services in this program are not capped, but must be approved by the service recipients Inter-disciplinary Team (IDT), the IDT is made up of service recipient, service recipient’s guardian/family, DDSD Case Manager, service provider staff and any other professional knowledgeable about the health or care of the service recipient; and then the IDT recommendations are reviewed and approved or denied by the State office of DHS/DDSD. Prior authorization is required for all services provided.
Oklahoma's first Home and Community-Based Waiver for people with intellectual disabilities was approved in 1988. This waiver, now known as the Community Waiver, includes a comprehensive array of services including residential, employment and habilitation services and supports for individuals 3 years of age or older. The Community Waiver provides services appropriate to the individual's needs. Using a person-centered planning approach, the individual's team assesses the needs of the individual and develops an annual plan of care to address those needs.
In accordance with the needs of the individuals to be able to meet the needs and be transported to doctor appointments, work, shopping, community outings etc.
